ORDERING INFORMATION

If it is for a home party you can give it to the hostess prior to her party so that she will get the credit for it. To order candles for an upcoming party, print out this order form, fill it in completely and give it to the party hostess prior to her party so that she gets credit for your order.

If the order is for a home party please put the hostess’s name in the appropriate box on the form. 

Shipping charges are as follows:

If shipped to your hostess with the rest of her party:

10% of your order,
$1.00 Handling Fee,
5.5% WI sales tax

If shipped directly to you:

10% of your order total,
$8.50 Handling Fee
5.5% WI sales tax if shipping within Wisconsin
